I am in network with HealthPartners, BCBS, Ucare, Atena, Medica, UnitedHeathCare, UMR and UBH. I currently accept private pay outside of those listed. You may also be able to utilize your out-of-network benefits, health savings account (HSA) or, flexible spending account (FSA) for sessions.
Because I cannot predict exactly what would be covered by your individual plan, you may contact your insurance company directly to verify your exact benefits or to answer any questions you might have about your mental health coverage.

My attendance policy is as follows:
•Arriving more that 15 minutes late to an appointment is considered a missed appointment.
•A 48-hour notice (business hours) is required if you are needing to cancel/reschedule so that someone else has enough time to schedule.
•The first cancellation with less than 48-hours in advance will be charged a $100.00 missed appointment fee.
•Any missed appointments after that will be charged a $150.00 fee.
•Missed appointment fee charges will be applied to your card on file at the time of cancellation.
•After 3 missed appointments, need of therapy services will be discussed and may be discontinued.
We all get ill, and emergencies happen that are unavoidable. I totally get that and will be as flexible as possible. If either of these situations arise, notify me directly as soon as possible. Please reply to this email with acknowledgment that you have read it. If you have any questions or concerns, feel free to email me at cindy.wiltgen@cmwtherapy.org
